Help At Home Coordinator Jobs

MAIN PURPOSE OF JOB:

To assist with handling all incoming requests for help, both by telephone and email, to listen to and record the needs of elderly and/or terminally ill people and their pets (ie petitioner needs).  To identify individuals outside our charitable remit and refer to other organisations that can offer assistance and always seek advice in the case of doubt.

Where help is required, to raise a computerised record to log the case, task volunteers nationwide by telephone and email to offer assistance to elderly petitioners with the pet care and reassurance needed in order to honour our mission statement of “Peace of mind for owners, love care and safety for their pets”.  Liaise with both petitioner and volunteer throughout to ensure the delivery of our service.  This post will require a good overall understanding of the needs of elderly people and their pets.

An essential occupational requirement of this role is to participate with on-call midweek evenings and weekends (this will be one full week in five).  Together with other members of the on-call team you will participate with on-call on a rota basis to help ensure and maintain our emergency telephone line during these times.

MAIN DUTIES AND RESPONSIBILITIES:

i. To assist in contacting volunteers nationwide by telephone and email and tasking by telephone as required to help petitioners with their pets.

ii. To listen to the needs of each petitioner and accurately record information for each petitioner and their pet by completion of a computerised case to record volunteers assigned and the service provided. 

iii. To ensure you record all of your communications onto the database in an accurate and timely fashion by way of comments on each petitioner case at the end of each conversation.

iv. To help maintain up-to-date records and mailing lists for petitioners, petitioners’ contacts and volunteers throughout the course of your work.

v. To ensure the appropriate delivery and quality of service provided to petitioners when you are handling a request for:-

a) Dog Walking

Task a minimum of two volunteers per case to assist with dog walking.  Contact all volunteers after their first dog walk to check and ensure they have started, log all responses onto the database, then complete the case.

b) Home Checks

When a potential long term fosterer is identified, you will be asked to task a volunteer to visit and carry out a home check.  Once the completed home check has been received, complete the case – (all home checks will be valid for 12 months).

c) Short Term Fostering (stf)

To initially gather as much information about the pet(s) as you can and how soon a foster is required.  Raise a case and log all details onto the database, begin a computerised search for a suitable volunteer to foster, arrange transport, if needed, to take the pet(s) to the volunteer and follow up 24 hours after the pet arrives to ensure all is well.  Keep in touch weekly.

If the pet goes home – arrange transport to take the pet back home, again follow up 24 hours after the pet has returned home with a phone call to the petitioner to ensure all is well.  If dog walking or any other assistance is needed, raise a new case and task the appropriate volunteers to either dog walk, offer Pet Care, reassure etc.

If the pet(s) goes into long term foster – refer to your line manager, once a decision is made, then follow the protocols for Long Term Fostering see below.

d) Long Term Fostering (ltf)

Raise a Long Term Foster Case, if needed, identify and chat to potential individuals, all of whom must already be home checked, run through the home check with the person to identify any changes, refer to your line manager, once the right individual is selected, arrange transport to take the pet(s) to their new ltf home.  Once the pet(s) have arrived, follow up 24 hours later to ensure all is well.

e) Pet Care

When required, task a volunteer to visit a petitioner and offer pet care, whether it is to administer medicine, groom, clean out a litter tray, bird cage, reassure, or any other pet care as agreed with your line manager as this is not an exhaustive list.

f) Talks

To task a volunteer to give a talk, attend an event, or represent The Trust, but, always take advice from your line manager with regard to appropriate volunteers for these tasks.

g) On-call

To ensure delivery of our service by participating in on-call and helping to maintain our emergency telephone line.  Together with other members of the on-call team, on a rota basis to help answer on-call emergencies this will be one full week in five – Monday to Monday including the weekend encompassing bank holidays, if applicable.  A day in lieu will be given for working bank holidays.

Upon completion of an entire week on-call and following a debrief with your line manager, a rest period of 1 day will be given to you immediately after ie Tuesday. 

On-call is for emergencies which will require an urgent response, for example if an ambulance is waiting to take someone to hospital, your urgent response is vital to provide peace of mind for the owner, love care and safety for a frightened bewildered pet.

v. To liaise with all other members of The Trust to help maintain a quality service.

vi. To contribute to the team by helping to update volunteer and petitioner cases in the course of your communications.

vii. To help maintain filing and filing systems related to your work.

viii. To liaise with all other staff of The Trust in a flexible manner.

ix. To liaise with all petitioners and members of the general public in a polite and helpful way.

x. To be familiar with and adhere to procedures and protocols, disciplinary and applicable rules and ensure compliance with legislations (Health and Safety, Data Protection, especially the privacy of members, petitioners and volunteers alike).

xi. At all times to ensure and maintain a compassionate, professional and efficient public image for The Trust.

The Healthy Ageing Project will address specific challenges within the community, while actively promoting healthy ageing in practical and effective ways across our network.

We will focus on the strengths and expertise of our member groups to promote engagement with Ireland's unique music, dance, sport, language and culture.

We will work together with our members throughout this three-year journey. Together, we will develop, launch and promote a project geared towards improving health and wellbeing, reducing health inequality and providing strategic direction for national health programmes and policy recommendations for the Irish community.

This post is part time (17.5 hours per week) and will be based in the North of England, Liverpool, Leeds, Manchester etc.

The Ripple Pond is a charity that supports adult family members of physically and/or psychologically injured British Armed Forces personnel and veterans. Many of these individuals and family members are hard to reach and often isolated.

Your role will be to ensure that more and more of the military community, professionals and other interested parties know who we are and how to access our support.

You will also help to organise our exciting programme of online talks and activities for our Members (service-users) to help ensure they are well-informed, involved and feel part of a supportive community. Making sure that Members are always involved in generating ideas and production.

We are looking for someone who is dynamic and determined, but you also need to be invested in our mission as a charity and care about people.

This role is about;

• working with a wide range of groups and individuals to help raise awareness of the charity and to create opportunities to identify new Members (service users);

• being responsible for networking, identifying, and establishing partnerships with key individuals and organisations and nurturing existing relationships;

• developing and rolling out strategies to engage with hard-to-reach and isolated military carers and families;

• coordinating a range of activities (mainly online) to deliver information to Members, potential Members, referrers, and other stakeholders. These will include online talks from staff, Members and other “subject matter experts”; and

Although the role is home-based, you will not be working in isolation; you will be supported by the managers and workforce.

C4WS Welfare Coordinator Advert 2024:

C4WS Homeless Project (registered charity, no 1189622) which provides support to people experiencing homelessness year-round, through a well-established winter night shelter in Camden, a lunchtime club, Jobs Club, mentoring scheme, hosting scheme and various other activities. We are passionate about helping people rebuild their lives after homelessness and working towards living independently.

What We Do:

Every winter, we work with a number of churches in the London Borough of Camden who offer their venues to us to run a night shelter. We operate from a different venue every night of the week, and run one circuit, accommodating 16 guests.

We provide camp beds and offer a hot evening meal. Our guests leave in the morning after a hot breakfast, ready to attend activities and meetings in our offices in Euston.

Advice and Support

Whilst the shelter operates seasonally, every winter, our team works year round, dedicated to offering advice, advocacy and practical support. This can be accessed by former night shelter guests, and via our weekly drop in, Friday Club. Guests can also access other services provided by C4WS including lunch, laundry and showers.

The Role:

Our small, dedicated team is looking for a Welfare Coordinator, to deliver holistic welfare support to all those who access our services. You will work alongside another member of the team to ensure that guests are supported both during and after their stay in the C4WS night shelter to ensure the stability of their move on.

You will possess a passion for working in the homeless sector, supporting those experiencing homelessness to rebuild their lives and ideally have direct and relevant experience of working with this demographic, or similarly vulnerable and/or disadvantaged groups.

As well as providing Welfare Support, you will also be a presence at the shelter for up to two evenings a week during the shelter season (November – March) including weekends, and you will need to lead a team of volunteers to ensure the safe running of the shelter on those occasions.

C4WS is a small charity, that delivers an outstanding service to those who access our support. It is thanks to a wide network of referral partners, corporate and individual volunteers, plus the support of several religious communities in Camden, who we have worked with over the last 20 years, that we can have such a positive impact on a person’s life. You will become part of that network, and learn more about the sector, as well as being focused on the current landscape of homelessness in the Borough of Camden.

The role

The post of Project Coordinator is critical to the work we do and this is a great opportunity to make a real difference to improve health and care services and make them work better for the patients and local people of our city.

You will be responsible for ensuring the delivery of a variety of patient-focused projects on different topics, against quality, time and cost constraints. You will manage projects from inception to delivery including publicising, following up on any recommendations, and promoting the impact that we make. As we are a small team you will also be expected to contribute collectively to a shared workload to keep the team working effectively.

Some of the projects which Healthwatch Brighton and Hove projects have delivered in the last year include:

• Improving home care services
• Improving dementia services
• Improving maternity services

We have a strong team of employees and volunteers and you will be supported by our Head of Research and CEO.

Work Location

Our office is at Community Base, 113 Queens Road, Brighton. We’re a home-based team, but hold team meetings at the office every Monday and you will be expected to work in the office on this day. There is also an option of office working on Thursday and hot-desking in either the office or Community Works’ office space on other days.

We will provide line management, technology and equipment support to enable home working. Applicants must be prepared to undertake this role in either form of work setting.

Main responsibilities and duties:

1. To be responsible for all aspects of planning and delivering a portfolio of time-limited projects. Typically, this will include:

• scoping evidence-backed projects which support our wider strategic aims
• contributing to written project plans
• Using Excel or SPSS to analyse data
• leading on undertaking service review fieldwork including interviews, ‘Enter and View’ visits (where we visit services in person) and Mystery Shops of our local hospitals

About the role

As a Lived Experience Coordinator, you’ll deliver a programme of involvement activities with the aim of ensuring the views and experiences of individuals with lived experience of bad housing or homelessness informs all of Shelter’s work. We’ll also rely on you to deliver involvement activities across the organisation which will shape the ongoing design, delivery and governance of our work. People with lived experience will be involved in a range of ways too, including involvement groups, taking part in events within local communities, co-designing services and campaigns, sharing stories and speaking out about the housing emergency. The overriding aim is to work closely alongside people with lived experience to influence and steer the direction of Shelter’s work, so that lived experience informs everything we do.

About you

To succeed, you’ll need the ability to facilitate involvement groups and other activities to involve people with lived experience in Shelter’s work while ensuring a safe and inclusive space where everybody can contribute. The ability to plan and co-ordinate involvement activities with local teams, with several activities running during the same time period, is important too, as is a flair for developing successful relationships with colleagues across a large organisation, as well as with local organisations and partners. Some experience of presenting information verbally and in writing is also required, as is proficiency using IT tools to carry out your day-to-day work, including case management systems, Microsoft Office, internet, email and online meetings.

We are looking for a Creative Expression Coordinator to line manage and support the delivery of Creative Expression programmes including Arts & Crafts, Music, Creative Expression and our newest provision of drama. You will work with the Independent Living Skills and Creative Programmes Manager to ensure we deliver a creative programme that maximises our students' potential, makes the best use of local partnerships, enables students to develop their creativity and increases wellbeing and communication skills.

Share is a registered charity and a centre for training and wellbeing. We provide a range of programmes helping adults with learning disabilities, autism and other support needs become more happy, healthy, and independent. Main responsibilities 

• You’ll manage the delivery of our creative programmes at various locations looking to ensure they are delivered to the highest standard
• You will ensure all sessions are inclusive challenge students, and where possible, use digital technology and learning outside of Share to embed skills 
• You will line manage the Creative Expression Tutors supporting them in developing project objectives, conducting one-to-one and appraisal meetings and reviewing and agree project resourcing 
• You’ll monitor the quality and impact of the creative programmes, their outcomes and delivery

• You have experience in planning and coordinator inclusive activities with a focus on the creative arts 
• You have experience in coordinating, planning and delivering sessions for adults that enable people to acquire new skills, both social and practical

A fantastic children’s charity are looking for a Children's Service Coordinator based in Gloucestershire.

Salary: up to £37,300 dependent on experience and qualifications.

As Children’s Service Coordinator you will provide support to the Registered Manager to help launch and run a newly established team, in a brand-new solo home in Gloucestershire.

You would be based in a solo placement home in a beautiful setting in Gloucestershire, the home is for a young person with complex needs.

Key responsibilities include:

• Managing and leading a team of professional social care staff. 
• You may be required to step up as deputy to the Registered Manager
• Creating a strong and high achieving team.  
• Liaising with our key Stakeholders within and outside Action for Children. 
• Supporting the Registered Manager in data collection and analysis. 

The successful candidate will be passionate about making a positive difference in children's lives and ideally will have previously worked as Deputy Manager (preferred but not essential). Audit Project Coordinator

The National Neonatal Audit Programme (NNAP) Project Coordinator is an instrumental role for the College in which you will support the delivery of a high-quality national clinical audit along with supporting improvements in neonatal care for babies and their families.

As Audit Project Coordinator, you will coordinate the collection of high-quality audit data, and be responsible for coordinating sub-projects and spotlight audits.

Acting as the first point of contact for audit process and data enquiries received from hospital units, regional network leads and other stakeholder organisations, you will take responsibility for responding to each enquiry using your detailed knowledge of the audit methodology.

You will also respond to queries relating to the NNAP methodology and support paediatric services within NHS Health Boards, Trusts and Networks to understand and use NNAP results to inform quality improvement activities, as well as monitoring NNAP participation rates to define, execute and promote improvements to data submission processes.

Reporting to the NNAP Project Manager, you will conduct testing and quality assurance of audit software systems, liaising with software developers to evaluate, improve and update online data submission and reporting processes, whilst preparing updates to project webpages, user guidance and promotional materials for audit submission and participation. In addition, you will coordinate project webinars, workshops and national events for the NNAP and represent the NNAP at conferences to help promote NNAP activities.

You will also prepare and disseminate audit outputs and publications for different audiences, including audit commissioners and funders, children, young people and families, and NHS services participating in the audit.

Degree qualified or with equivalent experience in a mathematical or numerate discipline, you should have proven experience of producing high-quality written reports and promotional information, suitable for a range of audiences.

With strong critical thinking and problem-solving skills, a high level of numeracy and the ability to interrogate large, complex datasets in Excel or statistical packages, you should have a background of working in a health-related or research setting.

Your project management experience along with your proven organisational skills and your ability to adopt an orderly and precise approach to work should ensure you are able to take personal responsibility for your own projects.

Demonstrable experience of engaging and communicating with stakeholders effectively, along with a proven understanding of information governance, data protection and confidentiality processes are essential.

A project management certification/qualification, knowledge and experience of the use of statistical packages, and a general understanding of a clinical audit and the reasons why clinical conditions are audited locally and nationally, would be desirable.
